The photo is flipped left to right (the inlet is actually to the right when you look at Revel from the ocean). Does this happen for all pics?

In the process you flip the picture upside down (vertical) before adding the polar coordinates distortion filter
Perhaps you can also flip it horizontally too.
I haven't tried it. Without the vertical flip... It makes it look like a tunnel instead of an earth

Well with very lest 6/7 flight of my p2v+ experience I do it complete assumption method to shoot such Panoramas.
I use Photoshop and PtGUI to stitch being in 360 panorama industry for more than a decade it gave me ease in pain to do this.
