Sorting out crashes

Because we have a mix of good and bad expierences, lets have pilots that crashed and believe it was drone error post, date purchased, model, firmware, monitor used, mods, and less than 5 word discription.

Example:

05/30/2015 professional 1.2.6 ipad mini 2 none fly away.
 
I can see logging dates, hours, repairs, etc. for those who may persue future certifications but what does a fly-away mean in your example? How is that helpful?

Fly-aways are at best Verbs not Nouns as there is always an underlying condition(s).

By the time you collect all the information when is there time to fly? ;)

I'm not really concerned why it crashed. I'm more interested in what hardware was used and dates of the drones
