Good morning,

We are working to compile a list of verified UAV incidents - crashes, injuries, privacy violations, etc. We would greatly appreciate the community's assistance with this project.

Good morning,

Why do you think I am trolling? I am a cyber security professional with 10+ years in cyber investigations, a rated pilot (fixed and rotor), and a UAV pilot (3 DJIs and one E384.) I am legitimately interested in learning more about accident rates and other incidents relating to UAVs.

Here's one for me:

On Tuesday, August 9th, 2014 at approximately 11.30am, after having applied new firmware from DJI, I inadvertently took off without recalibrating the gimbal. As result, the gimbal tilt was inoperative and the contents of the SD card were declared trashed.

This was reported to the FAA and they have subsequently labelled it "Unconfirmed sUAS near miss with passenger jet in Los Angeles".

I have plenty more if you're interested.
 
I would like to report an incident on a privacy issue. Last week while flying at a nude beach in Laguna CA someone was taking pictures of my drone.

Yesterday I went through a few pre flight checks but after that, no matter what I did, I could get the motors to start. After some in depth analysis, it turned out I had left the remote controller at home.

Would you like me to add this as a "UAV Incident" ?
 
I had a scheduled take off time set for yesterday after Easter Brunch but due to one to many Bloody Mary's and not meeting the required FAA chill period and knowing my BAC was over 0.02% I cancelled flight in FAA compliancy.

This incident also warranted a report filing to the Bureau of Transportation Statistics due to missed departure time.

Sorry I'm not sharing my 'incidents' with anybody in case I might incriminate myself - not that I have ever had one. Once I have my permit, and I have one, they will be sumbitted to the CAA on Occurance Report Form SRG/1601.

I'm being serious, there is no reason at all for anyone to submit them you. To be honest I dont think you will get any response from forums. Ask yourself the question why would anyone want to publise an incident they had? Admitting "crashes, injuries, privacy violations, etc" as you stated. You tell us you are a professional but asking for incident reports like this does not sound that professional to me.

Im the Lord Mayor of London by the way...not, but I might be... :)

At first I took pity on the OP because of the childish responses.
But ... after reading through his report containing a list of all of the commercial or civilian UAV incidents reported to or identified by our group and see that the list of list of verified UAV incidents - crashes, injuries, privacy violations, etc already collected is not much better. It's simply a collection of (sometimes questionable) media reports of the kind journalists love to paste onto any drone-related story to pad it out.

Here's a good example ... http://boingboing.net/2014/06/25/drone-firm-to-naked-woman-no.html#more-314152. Nothing happened, no incident - it even explains that in the article but it's filed away here with the other verified UAV incidents - crashes, injuries, privacy violations etc.

This isn't a good indication of the quality of the OP's effort to gather accurate data or the business he advertises as providing the very best in sUAS investigation. There's nothing at all professional about the database so far.
 
The other day whilst flying I sharted at OH9:30 hours.

The incident resulted in no damage to the drone and no bystanders were injured. I finished the flight and proceeded to hand catch as normal however, I do feel my form may not have been up to my usual standards.
