Just watched the Video, made me sick, floating around then, bam we are going down. Wow, it sucks..

I know it's a bit far-fetched and I didn't intend this to be mean-spirited but, OP, you didn't execute the CSC by mistake, did you?

Assuming this is truly a hardware fault, how often do these types of failure happen?

Nah, wasn't even using the controls at the time as it was setting up for an orbit...I think the initial yaw was where it was turning to face poi.
