Possible causes for sudden flip over

Hi all
I recently had my phantom randomly decide to flip over and land upside down on the ground.Has anyone else had this happen ? Or does anyone know what may have caused it ? .Im about to change the power block and data pins as i was getting a battery communication error message on and off.Not sure if that had anything to do with the flip ?

I had mine flip on my second day, and it was my second crash. The first crash was a semi fly away due to my not letting the GPS and Home Point get set properly. That crash loosened an ESC connector. When the ESC became disconnected the next day, the Quad flipped over, as one of the Motors stopped, which will always lead to a flip.

I have checked all wires inside and nothing seems to be wrong.Will be flying it close for a while next time don't want it to flip like that again at height.
