Phantom CRASH / motor problem?

My Phantom crashed instantly.

What happens?
* Right after I take off, it hovers for a second or two and clashes.
* when it crashed it seemed to me that one specific side motor fail to work.

What did I do before it happened?
* I tightened the screws holding the motors.
* I open the phantom body to unplug tarot cable to Naza.

I did a test in the room without propeller and all motor runs like should. I at least don't see the difference.
Do you think I have a dead motor?
How do I know if it is dead?

There was a problem with the firmware whereas when you do a CSC 2 motors would spin faster than the other 2. This was fixed in firmware last month so make sure you have the latest firmware.
