Major fly away fail today during 4th flight

1st day flying my P3 and it flew away immediately after initiating auto take off (4th flight of the day). Camera is intact somehow, but getting an ESC status error. One of my motors was damaged and not spinning freely like the other 3.

Really fortunate nobody was hurt. Any ideas about what might have caused this?

Trying to figure out how to upload my flight record from my iPad.

Not until you give us some info to work on.
Could have been lots of things. If you replay the flight from the flight log it might give some clues.
