Has it happened again since, with same or different battery?

It has not. Tested the battery again with two flights recently and all is good. It was random.

Thanks for all the help everyone, great to have resources like this. I updated firmware on remote, phantom and all batteries tonight, will also fly a bit less aggressively in the future as well.
