Sounds like something in the area might have been interfering with the signal from your transmitter, sending erratic signals to your quad causing it to fly off and crash.
I wouldn't fly in that area any more.

I'm wanting to add FPV to my 1.1.1 and either put in the iOSD at the same time or a little later.

Tie one end of the string around the battery cable coming out of the Phantom. Using a piece of tape, tape the other end of the string to the inside of the battery compartment door.
The string is to fish out the battery cable when it gets too far into the interior of the Phantom.
